2008-2009 HIV Care & Prevention Combined Comprehensive Plan
The New Hampshire (NH) Human Immunodeficiency Virus (HIV) Care and Prevention
Comprehensive Plan (Plan) is developed as a collaborative effort of the NH HIV Community Planning
Group (CPG) in partnership with the Department of Health and Human Services' (DHHS) Division of
Public Health Services (DPHS) STD/HIV Section, stakeholders and other HIV care and prevention
service providers.

The updated 2008-2009 combined plan serves as a guide for HIV care and prevention services in New Hampshire in conjunction with the Statewide Coordinated Statement of Need (SCSN). The combined plan and the SCSN document support maximized care and prevention coordination, integration and effective linkages across the Ryan White Program Parts A-F as well as other federal and state funded programs and community based programs.
This document highlights the accomplishments of the NH HIV CPG and focuses on the last prevention planning year. The document reviews to the goals and objectives for HIV care and prevention planning that were established in the previous planning cycle and adopted for the 2008-2009 planning cycle.
The NH HIV Combined Comprehensive Plan is a dynamic document that is updated regularly by the NH HIV CPG and the NH DHHS STD/HIV Section based on the most recent available data, current services and the requirements of the CDC and HRSA.
