Comprehensive Plan

The New Hampshire (NH) Human Immunodeficiency Virus (HIV) Care and Prevention Comprehensive Plan is a collaborative effort of the New Hampshire HIV Community Planning Group (NH HIV CPG) in partnership with the Department of Health and Human Services' (DHHS) Division of Public Health Services (DPHS) STD/HIV Section, stakeholders and other HIV care and prevention service providers.

The plan documents that collaboration to ensure the services provided under the 2004-2007 NH HIV Care and Prevention Comprehensive Plan are consistent with the goals and objectives identified in the Statewide Coordinated Statement of Need (SCSN) based on epidemiologic data, the resource inventory and gap analysis, and the current needs assessments conducted during the 2004/2005 planning year.

The 2004-2007 combined plan serves as a guide for HIV care and prevention services in the state of New Hampshire. The NH Statewide Coordinated Statement of Need, in combination with the guidelines put forth by the Centers for Disease Control and Prevention (CDC) in the HIV Prevention Community Planning Guidance, and the goals and responsibilities identified for Ryan White CARE Act grantees by Health Resources and Services Administration (HRSA), were used to identify and address HIV care and prevention planning needs and efforts statewide in NH.

This 2005 document reflects the activities and efforts of the NH HIV CPG in partnership with the NH DHHS during the first year of the three-year planning cycle and provides an update to the 2004-2007 NH HIV Combined Comprehensive Plan. During the year-long process, the NH HIV CPG, following the eight steps of the planning process, determined what had been accomplished during this planning year and what still needed to be completed within the three-year planning cycle time frame. This document also highlights the future objectives of HIV care and prevention planning across the state for the remainder of the 2004-2007 planning cycle.
